South African Doctors Propose NHI Alternative with Medical Aids Intact

The Universal Healthcare Access Coalition (UHAC), representing South Africa’s leading healthcare associations, has presented an alternative to the government’s proposed National Health Insurance (NHI).

The coalition’s proposal seeks to integrate public and private healthcare systems, focusing on fiscal sustainability and maintaining medical aid schemes.

The NHI, championed by the African National Congress (ANC), aims to establish a government-managed fund to provide universal healthcare access. However, the proposed framework would diminish the role of private medical aids, limiting their ability to cover services offered by the NHI.

Critics, including the UHAC, argue that the NHI is “fiscally and institutionally unimplementable.”

UHAC’s Alternative Proposal

The UHAC published a report titled Universal Healthcare Access for South Africa, outlining roadblocks and solutions for a sustainable healthcare system.

Key Issues with the Current System:

1. Public Sector Challenges: Governance failures, resource inefficiencies, and declining service quality.
2. Private Sector Strain: Rising costs and regulatory gaps limiting access and sustainability.
3. Policy Vacuum: A 20-year gap in strategic health reform, worsening conditions for both public and private sectors.
4. Unbalanced Financing: Over-reliance on tax-based funding without adequate private contributions.
5. Political Influence: Governance compromised by political oversight.

Proposed Reforms

1. Separation of Pooling and Purchasing:
   • National-level pooling ensures equitable resource distribution.
   • Decentralized purchasing empowers local authorities and public hospitals to tailor healthcare delivery to community needs.
2. Strengthening Public Healthcare Services:
   • Grant autonomy to district health authorities and hospitals for workforce planning and procurement.
   • Implement governance frameworks to improve efficiency and service quality.
3. Expanding Contributory System:
   • Mandatory medical scheme membership for higher-income earners.
   • Financial support for low-income contributors.
   • Risk equalisation and social reinsurance to prevent inequalities in medical scheme coverage.

Moving Forward

The UHAC’s alternative offers a balanced approach, integrating public and private healthcare systems to achieve universal access, quality care, and financial sustainability.

“By addressing governance failures and ensuring financial sustainability, the framework upholds the constitutional right to healthcare for all South Africans,” the coalition said.

The proposed reforms, if adopted, could provide a viable pathway to achieving universal healthcare access without dismantling the private medical aid industry.
